California Workplace Violence Prevention Plan (WVPP)
A WVPP is the written document required by SB 553 that outlines your company's procedures for preventing and responding to workplace violence. Here's what it must include.
Required WVPP sections
Under California Code of Regulations Title 8, Section 3342, your WVPP must include the following elements:
- Responsibility: Names or job titles of persons responsible for implementing the plan
- Reporting: Procedures for employees to report workplace violence incidents and threats
- Compliance: Procedures to ensure compliance with the plan and the involvement of employees
- Communication: How the employer will communicate with employees about workplace violence and respond to questions
- Hazard identification: Procedures for identifying and evaluating workplace violence hazards
- Hazard correction: Procedures for correcting identified hazards in a timely manner
- Training: Training content, frequency, and documentation for all employees
- Recordkeeping: Procedures for maintaining the violent incident log, training records, and hazard assessments
- Annual review: Procedures for reviewing and updating the plan at least annually
- Post-incident response: Procedures for responding to and investigating incidents
Violent incident log
SB 553 requires employers to maintain a violent incident log for every workplace violence incident. The log must include:
- • Date, time, and location of the incident
- • Type of incident (Type 1–4 violence classification)
- • Description of what happened
- • Who was involved
- • Consequences (injuries, property damage)
- • Corrective actions taken
The log should avoid unnecessary personally identifying information to protect employee privacy.
Types of workplace violence
Cal/OSHA classifies workplace violence into four types:
- • Type 1: Violence by a stranger (e.g., robbery)
- • Type 2: Violence by a customer or client
- • Type 3: Violence by a coworker or former coworker
- • Type 4: Violence by someone in a personal relationship with an employee
