California Workplace Violence Prevention Checklist
Use this checklist to verify your California business has the essential SB 553 compliance elements in place.
Written Plan
- Written Workplace Violence Prevention Plan exists and is accessible to employees
- Plan identifies who is responsible for implementation
- Plan includes procedures for reporting workplace violence
- Plan includes procedures for communicating with employees
- Plan includes hazard identification and evaluation procedures
- Plan includes hazard correction procedures
- Plan includes post-incident investigation procedures
- Plan includes annual review procedures
Hazard Assessment
- Initial workplace violence hazard assessment completed
- Risk factors identified (public-facing, cash handling, late hours, etc.)
- Hazard correction log maintained
- Corrections tracked to completion with verification
Incident Log
- Violent incident log template ready
- All incidents documented with date, time, location
- Incident type classified (Type 1–4)
- Corrective actions documented for each incident
- Privacy-protected fields used appropriately
Training
- Initial training completed for all employees
- Annual training scheduled and documented
- Training records include acknowledgment
- Training covers reporting procedures
- Training covers recognizing warning signs
Recordkeeping
- WVPP plan document stored and accessible
- Hazard assessment records maintained
- Incident logs maintained
- Training records maintained
